Personal Health Support

If you or a covered family member is managing a chronic condition or navigating complex medical care, you may be paired with a Personal Health Support Nurse. These experienced nurses serve as your dedicated point of contact—offering education, guidance, and coordination of care.

Key Services include:

  • Admission Counseling: Help preparing for surgery and recovery.
  • Inpatient Care Management: Support during hospital stays to ensure effective treatment.
  • Readmission Management: Post-hospital support for high-risk conditions, including help with medications, follow-ups, and discharge instructions.
  • Risk Management: Assistance with chronic/complex conditions, including access to specialists and coordination of equipment and care.
  • Cancer & Kidney Management: Specialized nurse support for individuals with cancer or advanced kidney disease.

Decision Support

UHC’s Decision Support program provides the tools and guidance you need to navigate your treatment options confidently. For certain conditions and procedures, you’ll have access to health information, provider recommendations, and nurse guidance—all designed to support smarter healthcare decisions.

Support is available for:

  • Back pain
  • Hip & knee replacement
  • Prostate conditions & cancer
  • Benign uterine conditions
  • Breast cancer
  • Coronary disease
  • Bariatric surgery

Disease Management

If you’ve been diagnosed with heart failure, coronary artery disease, diabetes, asthma, or COPD, you may qualify for a free disease management program. These programs connect you with a registered nurse who specializes in your condition and provide helpful tools to support better health.

What’s included:

  • Condition-specific educational materials
  • Online resources for self-management
  • Nurse outreach for medication and symptom management
  • Support for doctor visits, care coordination, and lifestyle changes
  • Guidance on community resources, safety at home, and caregiver support
