Accident Insurance
Accident insurance from MetLife is an optional supplemental insurance policy designed to provide a financial benefit to help cover the costs of being injured in an accident. Accident insurance plans pay benefits based on the type of treatment needed or expense incurred when the insured is injured in an accident. The more extensive the treatment(s), the more the policy will payout. Benefits payouts do not depend on participation in other employer insurance offerings. With MetLife, you’ll have a choice of two plans (called the “Low Plan” and the “High Plan”) that provide payments in addition to any other insurance payments you may receive.

Critical Illness Insurance
Critical illness insurance from MetLife is a special form of insurance that provides policyholders with a lump sum payment in the event of a catastrophic diagnosis. The policyholder can use the money they receive however they like. Many put it toward medical bills while others use it to replace lost wages while they go through recovery. Types of illnesses covered included are (but not limited to) heart attack, cancer, organ transplants, Parkinson’s Disease and ALS. Employees are offered a choice of the following benefit amount options: $15,000 or $30,000.
MetLife will provide an annual Health Screening Benefit of $100 per calendar year per covered family member for taking one of the eligible screening/prevention measures. Claiming your benefit is easyopens the PDF in new tab.

Hospital Indemnity
The benefit covers planned (e.g., maternity) or unplanned (e.g., accident or illness) hospitalizations. This benefit is paid directly to you, and you can use it to help cover out-of-pocket expenses related to your hospital stay, such as deductibles, copays, coinsurance, or other costs like transportation or childcare.

How to Submit a Claim
Submit an Accident, Hospital Indemnity, or Critical Illness claim online. It’s as simple as 1-2-3:
- Register at mybenefits.metlife.com to view your certificate of insurance and to initiate your claim* or call 866-626-3705 to obtain a claim form*
- Answer some questions about your claim and upload your medical documentation to support your claim. The whole process takes just minutes!
- Visit MyBenefits frequently to check claim status, letters and benefit payments.
- A MetLife claims specialist will review your information, request any additional medical information (if necessary), and notify you in writing of a claim decision.
*For Critical Illness claims, a Physician Statement, which is available on MyBenefits, needs to be completed by your physician.
